We are growing in the Hartford region and are seeking a Full-Time RN Case Manager to join our team.


Position Overview

The RN Case Manager is responsible for managing a caseload of home health patients, coordinating interdisciplinary care, and ensuring clinical excellence, compliance, and outstanding patient experience.

This position offers autonomy in the field with strong clinical support from leadership.


Schedule
  • Monday through Friday

  • Weekend on-call rotation (shared among clinical team)


Responsibilities
  • Perform Start of Care (SOC), Recertifications, ROCs, and routine skilled nursing visits

  • Develop and implement individualized care plans

  • Provide patient and caregiver education

  • Coordinate care with physicians and interdisciplinary team members

  • Ensure compliance with Medicare regulations and agency policies

  • Complete accurate and timely documentation in EMR

  • Participate in case conferences and team meetings


Qualifications
  • Active Connecticut RN license

  • Minimum 1 year nursing experience (home health preferred)

  • Strong clinical judgment and assessment skills

  • Excellent time management and communication skills

  • Valid driverโ€™s license and reliable transportation

  • CPR certification
