Position Summary
The Case Manager / Career Coach supports participants in the DC TANF program by providing comprehensive case management, career coaching, education and training guidance, and barrierโremoval referrals. Case Managers maintain a caseload of 50โ60 customers and are responsible for meeting monthly performance goals in a fastโpaced, resultsโdriven environment. This position is inโoffice, five days per week.
Key Responsibilities
- Conduct intakes, assessments, orientations, and motivational coaching
- Develop individualized education, training, and employment plans
- Monitor customer progress and maintain weekly outreach
- Maintain accurate, compliant electronic case files and documentation
- Enter and track data in company and funder databases
- Prepare weekly and monthly caseload reports
- Connect customers to community resources to address employment barriers
- Build positive relationships with customers, partners, and colleagues
- Home Visits - At least twice in the month
Qualifications
- Bachelorโs degree in social work, Psychology, Business, or related field
- 2โ5 years of experience in case management, workforce development, or social services
- Strong organizational, administrative, and documentation skills
- Excellent written and verbal communication skills
- Ability to work with diverse populations with high barriers to employment
- Proficiency in Microsoft Office and database systems
- Results-driven, adaptable, and customer-focused
Pay Rate: $24.04/Hourly
