Position:  Licensed Nurse (LN)

Department:  Care

Supervisor:  Resident Care Director (RCD)

IMPACT STATEMENT

The responsibilities of the LN include, but are not limited to, fulfilling the company’s mission, vision, and core value statements while achieving census, revenue, and profit goals as well as building and maintaining valuable referral relationships within the senior care community.  He or she is responsible for offering the highest quality of care and complying with AHCA, county, and city regulations.  The LN’s vital job function is to make sure that revenue, expense, and profit goals are met continuously.  The LN works as a liaison between residents, resident families, and team members.

DUTIES AND RESPONSIBILITIES 

Responsibilities include, but are not limited to, the proper medication management and the essential care of the residents by working with all departments, the medical community, families, and the administrative team.  The LN fulfills the residents’ needs with continuity and while adhering to the company policies, state, and county regulations.  He or she is involved in the proper management of the medication room, med-pass techniques, MAR, resident charting and all other aspects of medication and the care of the community’s residents. 

Nursing and Care

  • Maintain resident’s files in an organized fashion the follows company policy and complies with state regulations
  • Review resident service plans, per AHCA regulations and company policy, and ensure they are being followed correctly by team members and outside services
  • Ensure residents’ ADL needs are safely and properly performed
  • Ensure residents’ supply needs are met by either POA or company
    • POA should generally provide clothing, linens, towels, incontinent supplies, personal hygiene products, necessary medications, special food requirements, and other supplies not covered in resident’s monthly rate
  • Communicate urgently with RCD anytime there is a significant change in physical, mental, or behavioral condition
  • Ensure that a personal visit occurs following a resident incident, accident, injury, or return from hospital/rehab
  • Notify physician and/or designated agent(s) of resident status/change as appropriate
  • Coordinates and/or supervises appropriate documentation for all changes in condition
  • Performs weekly review of medication administration records
  • Performs medication pass as needed
  • Provide residents assistance with ADL’s as needed
  • Reads staff communications and maintains familiarity with residents needs
  • Communicates changes in services plans to care team as the need occurs
  • Review physician’s orders to ensure accuracy of medication and treatment administration
  • Order necessary medications and treatments supplies for resident
  • Monitors any resident skin concerns weekly
    • Notify the doctor or nurse practitioner of new skin problems, changes of condition, and when skin conditions heal
  • Communicate urgently with RCD and other necessary health care professionals in the event of an incident and take necessary action to ensure the safety and wellbeing of the resident
    • Perform proper incident reports and action plans per company and state policy
  • Participate in shift exchange meeting daily and communicate critical information in the shift-change process
  • Assist your supervisor with resident move-ins to be sure accommodations are as desired and care is immediately implemented
  • Solicits feedback on an ongoing basis from residents and others to improve care services in the community
  • Provides appropriate actions to help resolve complaints in a timely fashion
  • Other duties as assigned by RCD and Executive Director (ED)

Administrative  

  • Assist RCD in developing and maintaining a current individual care plan for each resident
  • Assist with admissions, discharges, and transfers of residents
  • Assist in electronic medication administration and electronic health records and other audits as directed by RCD
  • Ensure the overall happiness, health, and safety of all residents
  • Build positive family and POA relationships to ensure customer’s satisfaction
  • Ensure compliance with all company, state, county, and city regulations
  • Assist with resident files to ensure compliance with all company, federal, state, county, and city requirements
  • Help realize that the Quality Management program is performed for best care
  • Work closely with outside services (i.e. – hospice, home health, PT/OT, etc.)
  • Build a positive-compassionate company culture
  • Enhance impressions, professionalism, and presentation of community team
  • Inspire, motivate, and be involved with the community team for optimal performance
  • Build unity, loyalty, and accountability among all team members
  • Assess the needs of the residents in the community and implement timely solutions to ensure that each resident’s needs are being met and that their care charges are sufficient to provide uncompromised care
  • Arrange and schedule transportation as needed and desired by the residents
  • Ensure that resident and family needs, requests, and complaints are addressed- Inform supervisor, in writing and verbally, of all family and resident complaints
  • Actively market community and establish good referral relationships, both in general, and with the many specific groups/individuals important to the community’s success
  • Perform tours and handle marketing calls/visits as needed

Leadership

  • Provide supervision of care team
    • Delegate administrative authority, responsibility, and accountability of directing care team
  • Ensure care is delivered as indicated in resident care plans
  • Assist and/or conduct orientation for new care team members
  • Identifies care team training needs and communicated those needs to RCD
  • Creates a positive environment, which encourages creative, positive problem solving and overall job satisfaction for team members
  • Develops a strong team culture which fosters interdepartmental cooperation
  • Demonstrate exemplary attitude, work ethic, and unwavering integrity  
  • Be effectively involved in the ongoing marketing effort of the community

Customer Service

Dining/Culinary

  • Contribute to the extraordinary dining experience of the community  
    • Know each resident’s dietary needs/restrictions, tastes, and preferences; work with the Culinary Experience Director (CED) and culinary team to ensure proper diet, resident’s satisfaction, resident choice, and that resident’s service plan is followed
    • Assist in the dining room during meals unless otherwise required
  • Help the community provide personalized meals three times daily as well as drinks and snacks around the clock

FINANCIAL EXPECTATIONS

  • Generate revenue to meet and exceed business goals
  • Minimize expense by ensuring community team payrolls are in budget
  • Minimize expense by ensuring supply expenses are in budget

  • Previous experience working with the elderly population as a nurse in the geriatric field

  • Must be honest, dependable, and exhibit unwavering integrity

WORKING PARAMETERS

  • According to schedule

  • Extra hours required in accordance with resident and shift needs
  • All overtime must be approved by ED
  • Clocking in and working, other than what is scheduled or approved by supervisor, is not acceptable

BENEFITS

    • Full Time Team Members (30+ hours per week) – eligible for benefits starting the 1st of the month proceeding 60 days of full-time employment
  • Paid Time Off (PTO) - for full-time team members (30+ hours per week)
    • PTO calendar year accrual applied each pay period
      • Level of accrual- 1.67 hours per pay period (1 week per year)
      • Calendar year cap of 40 hours
    • PTO is accrued from date of hire but cannot be used until after your 90-day introductory/probationary period
    • Accrued PTO is forfeited if employment is terminated, or employee resigns without proper written notice (14 days)
    • Time off requests must be initiated in team member’s individual iSolved account and approved by supervisor to received PTO.  Time off request(s) will be accounted for in one of two ways:
      • If there is a positive PTO balance, the missed days will be deducted from that accrued balance
      • If there is a zero PTO balance, the missed day(s) will be unpaid PTO time
    • If time off is taken without a written approval from supervisor, the time off will be deducted at a rate of 1.5x from the PTO balance or from employee’s salary (ED and/or HR Director will determine which)
      • Termination may result, at the ED and/or HR Director’s discretion, if time off is taken without proper approval
