Job description

The Director of Value-Based Care and Clinical Transformation is responsible for the overall direction, execution and management of the Value-Based Program, as well as execution of strategic clinical partnerships with providers. This role will report to the Associate Chief Medical Offer, Quality & Transformation.
This Director of Value-Based Care and Clinical Transformation will oversee the strategy development and execution of JHHP's Value-Based Program across all lines of business. The Value-Based Program is a key driver of provider engagement in quality performance, health outcomes and population health management.
  • The role involves VBC program design and negotiation, oversight of program operations, and provider engagement/partnership development.
  • VBC program oversight involves collaboration with finance and analytics on financial modeling and business planning, as well as provider contracting/network management on provider sourcing, collaboration and contract execution. There is additional collaboration with various quality teams and risk adjustment to ensure alignment with quality and HCC program goals.
  • The Director of Value-Based Care and Clinical Transformation will oversee the Provider Engagement & Clinical Transformation (PECT) team that manages provider relationships as it relates to VBC Program participation and building out strategic payer-provider partnerships that foster clinical transformation and promoting best practices for driving outcomes in quality and population health management.
  • Program operations responsibilities includes business ownership of the VBC provider reporting package; the Director will guide the reporting/analytics needs of providers to be support successful VBC performance, working with internal data/reporting teams and/or vendors on implementation and delivery.
  • This Director of Value-Based Care and Clinical Transformation position is responsible for maintaining an intimate knowledge of industry best practices in value-based care, awareness of JHHP's provider network and the evolution of value-based care in our market.

This Director of Value-Based Care and Clinical Transformation position will be charged with execution of strategic partnerships with providers as it relates to clinical transformation efforts to improve access to care, health equity, and adoption of innovative clinical models. This work is to closely partner with the ACMO and Line of Business Leads to develop concepts and approaches that support strategic priorities for the organization that require advanced provider collaboration.
Requirements:
Education:
Bachelor Degree in Business, Finance, Health Care Administration or a clinical field
Masters degree in relevant field preferred but not required
Work Experience:
Minimum 10 years of progressive experience in health care including experience with value-based care models, with strong financial and analytic skills.
Salary Range: Minimum 49.01/hour - Maximum 78.43/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
