Job description

Affinity Healthcare Solutions is looking for skilled, compassionate Registered Nurses (RN) and Licensed Practical Nurses (LPN) to provide shift care for a Peg-tube, trach and vent dependent patient in the 19090 zip code area of Willow Grove. This is a patient who needs round the clock care with consistent scheduling, offering an opportunity for nurses to provide one-on-one, high-quality skilled nursing care in a home setting.

Immediate start is available for qualified candidates who are passionate about patient care and reliability.

We use Axxess and hhaexchange.

Responsibilities
    1) Deliver skilled nursing care to trach/vent-dependent patients in their home
    2) Conduct thorough assessments and maintain accurate medical records
    3) Monitor vital signs and patient conditions, and report changes promptly
    4) Administer medications and other prescribed treatments
    5) Oversee fluid intake/output and maintain comprehensive patient charts
    6) Collaborate with the family andinterdisciplinary healthcare team to ensure optimal care
    7) Provide education, emotional support, and guidance to patients and caregivers
    8) Adhere to all clinical and regulatory standards to ensure patient safety and quality of care
Skills
    1) Active RN or LPN License in Pennsylvania (LPNs may apply to other postings)
    2) Trach and vent experience is required
    3) At least 1 year of clinical or home health experience preferred
    4) Strong critical thinking and assessment skills
    5) Familiarity with infection control protocols, nursing best practices, and safety standards
    6) Excellent communication and interpersonal skills
    7) Proficiency in documentation and use of electronic charting systems (Axxess and HHAeXchange experience a plus)
    8) Reliable, compassionate, and able to work independently in a home care setting
