Position Summary

We are seeking a dedicated and mission-aligned Advanced Practice Provider to join our integrated care team. The Advanced Practice Provider will provide high-quality, age-appropriate, and specialty-specific care to LLCHC outpatients in coordination with a collaborating physician. The APP will practice within their scope of licensure, certification, and education, and will work alongside a multidisciplinary team to deliver holistic care in alignment with LLCHC's faith-based mission.

The ideal candidate is a collaborative leader and clinician who thrives in a community health setting, is committed to serving diverse populations, and is passionate about eliminating health disparities through accessible care.

Key Responsibilities

  • Deliver high-quality, patient-centered primary care to individuals and families of all ages.
  • Manage clinical schedules efficiently while maintaining patient access and continuity of care.
  • Utilize evidence-based guidelines to support diagnostic and treatment decisions.
  • Document care in a timely, accurate, and compliant manner within the EHR (Epic experience preferred).
  • Serve as a clinical leader in your care team and actively participate in team-based care delivery.

Work Environment & Physical Requirements

  • Primarily clinic-based; may include occasional community outreach or telehealth.
  • Ability to lift up to 25 lbs, bend, stoop, and move intermittently throughout the day.
  • May require occasional evening or weekend hours based on patient needs.

Qualifications

Required Qualifications

  • Master's or Doctoral degree in Nursing or Physician Assistant Studies
  • Board certification in the relevant specialty
  • 2+ years of clinical experience in a healthcare setting
  • Strong communication and interpersonal skills
  • Experience with telemedicine services
  • Current and unrestricted state license to practice as an NP or PA
  • National certification from recognized certifying bodies (e.g., AANP, ANCC, NCCPA)
  • Current BLS certification (ACLS and PALS preferred).
  • Demonstrated commitment to serving diverse, underserved, or vulnerable populations.
  • Strong interpersonal and communication skills with a team-oriented approach.

Preferred Qualifications

  • Prior experience in a community health center or similar primary care setting.
  • Familiarity with EPIC or other EHR systems.
  • Bilingual (Spanish, Somali, or Arabic) is a plus.
