We are seeking a full-time Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to provide call coverage to our Post-Acute Facilities. This position is fully remote and can be performed from home.
Shift: Evenings

Qualifications:
  • Graduate of an accredited Physician Assistant or Nurse Practitioner program
  • Current national certification as Physician Assistant or Nurse Practitioner.
Responsibilities:
  • Provide clinical evaluation, treatment planning, and ongoing management for patients in Post-Acute Facilities during off-hours
  • Collaborate with interdisciplinary teams to ensure high-quality, timely patient care
