As a RN Admission Nurse at Triniti Home Health & Hospice you will provide a smooth transition for our patients by cultivating relationships and coordinating the admission process. Our ideal candidate thrives in a fast-paced environment, excels in multitasking, and has excellent customer service skills.

Position Type: Full-Time, benefit eligible position based from our Metro Branch in Edina.

Wage Range: $42 - $44 / hour depending on experience

Location: 4600 W. 77th Street, Edina, MN. 

RN Admissions Nurse Responsibilities:

  • Evaluates all patient referrals, manages admissions to home health care and completes OASIS. 

  • Ensures a smooth admission of patients by collaborating with the Case Manager. 

  • Maintains positive relationships with current referral sources.

  • Documents and maintains paperwork, referral records, and reports.

  • Performs other duties as assigned.

  • Anticipates & Orders equipment needed for patient’s episode. 

RN Admissions Nurse skills and requirements:

  • Must be a licensed RN in the State of Minnesota.

  • 2 + years’ experience in skilled nursing, the admission process, discharge planning, or management.

  • Strong knowledge of local, state, and federal guidelines related to home health care. 

  • Must be able to comprehend clinical and technical data.

  • Strong leadership, written and verbal communication skills.
