Who you are:

  • Current licensure by the California Board of Registered Nursing or Vocational Nursing
  • Current CPR certification
  • Two years in long-term care with prior charge nurse experience preferred
  • Experience with wound care and admissions strongly preferred
  • Knowledge of CMS regulations and long-term care standards
  • Strong assessment, organizational, and communications skills
  • Ability to work independently and as part of an interdisciplinary team

Job Summary:

Coordinates and completes clinical admissions for new residents in a timely and accurate manner.

Essential Job Duties:

  • Perform comprehensive nursing assessments upon admission
  • Review hospital records, discharge summaries, medication lists, and physician orders
  • Initiate and document care plans based on resident needs
  • Communicate effectively with physicians, interdisciplinary team members, residents, and families
  • Ensure all admission documentation meets regulatory and facility standards
  • Perform wound assessments, treatments, and dressing changes per physician orders
  • Ensure compliance with infection control, safety, and quality assurance standards
  • Maintain accurate and timely documentation in the electronic medical record (EMR)
  • Participate in care plan meetings and interdisciplinary rounds as required
  • Assist with audits, surveys, and regulatory compliance activities
  • Support quality improvement initiatives related to admissions and wound care
