PICKAWAY COUNTY COMMUNITY ACTION ORG., INC.

Position Title: Wellness Coordinator

Pay Grade: Varied upon experience

Reports To: Empowerment Services Director

Supervises: None

Standard Work Day: 8.5 Hours, Monday-Thursday, 7:30 -4:30

Job Summary:

The Wellness Coordinator will provide outreach services and wellness coordination and referrals to treatment programs. The Wellness Coordinator will provide education to clients and the community.

Serves as a consumer advocate, providing information and peer support for individuals in outpatient and inpatient settings.

The Wellness Coordinator will plan and implement activities that promote sober activities for individuals in recovery that align with the individuals' case plan.

Works closely with law enforcement, hospital, treatment, primary care, and emergency service personnel to identify individuals with OUD.

Meeting individuals where they are at, the Wellness Coordinator will educate individuals with OUD/SUD on available options, help coordinate care, and answer questions.

Required Qualifications:

  • High-school diploma or equivalency
  • At least two years of experience in social services, counseling, or related field.
  • Demonstrated knowledge of community services programs and community resources.
  • Valid Driver?s License and be insurable under agency policy.

Preferred Qualifications:

  • Undergraduate or associate degree or undergraduate work in human resources, social services, or related field is preferred.
  • Experience in prevention and/or recovery.

Competency:

  • Demonstrated knowledge of community services programs and community resources.
  • Good oral and written communication skills.
  • Command of all skills necessary to organize and manage multiple tasks.
  • Ability to communicate effectively with low-income, disadvantaged population
  • Excellent understanding and usage of Microsoft programs including Work, Excel, Outlook and PowerPoint

Travel:

Occasional nights, weekends or overnight travel out of county and in county travel may be required

Work Environment:

  • Extended hours may be required during critical workloads.
  • Majority of time spent in office setting within agency facility

Physical demands:

  • Requires standing, bending, reaching, sitting in working with office equipment such as computer, telephone, etc. Occasional lifting up to 50 pounds.

Essential Functions:

  • Determines individuals? needs by completing intake interviews.
  • Establishes case management plans by coordinating services such as connection to income and benefits, housing stability, employment counseling, and recovery coaching.
  • Serve as an advocate for individuals in outpatient and inpatient settings.
  • Assist program participants to become involved within the community.
  • Establish and maintain relationships with identified community partners.
  • Connects individuals to appropriate treatment options available.
  • Coordinate regular Wellness activities on an individual and group basis.
  • Coordinates transportation to treatment activities.
  • Monitors cases by verifying individual?s progress toward their goals.
  • Provide additional training and resources necessary to achieve and maintain goals.
  • Maintains individual confidential files including case notes; logging events and progress toward established goals.
  • Update job knowledge by participating in educational opportunities; reading applicable publications; maintaining personal networks; participating in regional meetings.
  • Responsible for tracking customer data, entering the data in the appropriate computer software or program.
  • Responsible for reporting program outcomes to the director and the funding sources.
  • Assist with grant application, administration and budget.
  • Plan and coordinate educational and awareness events for the community.
