Schedule: Monday-Friday 8a-5p with 2 holidays on call per year

The Hospice Resource Nurse (RN) is a key clinical support and education leader within our hospice team. This role strengthens the quality of patient care by providing on-site coaching, real-time clinical guidance, and targeted education to hospice clinicians. Working closely with Quality Auditors, Educators, and local leadership, the Resource Nurse identifies trends through chart audits and transforms those insights into meaningful improvements in practice, documentation, and regulatory compliance. This position serves as a mentor, subject-matter expert, and trusted resourcehelping clinicians deliver exceptional, compassionate hospice care while ensuring adherence to standards and workflows.

Essential Duties

  • Deliver in-person education and coaching on hospice standards, clinical guidelines, and documentation expectations.
  • Partner with local management to provide training sessions, skill validations, competency refreshers, and workflow reinforcement.
  • Serve as a clinical resource for symptom management, care planning, hospice eligibility, and regulatory requirements.
  • Review clinical documentation to ensure accuracy, completeness, and compliance.
  • Use chart audit data and dashboards to identify trends, gaps, and training priorities.
  • Collaborate with Quality Auditors to translate audit findings into actionable education and individualized coaching plans.
  • Provide constructive feedback to clinicians to improve documentation quality and timeliness.
  • Support agency-level quality initiatives in partnership with leadership, educators, and quality teams.
  • Assist with onboarding new clinicians through hands-on teaching and real-time support.
  • Participate in team meetings, case conferences, and process improvement activities.
  • Promote consistent application of hospice standards across the local team.
  • Model best practices in communication, care planning, and interdisciplinary collaboration.
  • Maintain current knowledge of hospice regulations, Medicare Conditions of Participation, and organizational policies.
  • Support workflow implementation and performance improvement projects.
  • Provide on-site presence to foster a culture of learning, support, and high-quality care.
  • Maintain confidentiality and comply with HIPAA, Medicare, Medicaid, and organizational policies.
  • Perform additional duties as assigned.

Position Requirements & Competencies

  • Graduate of an accredited registered nursing program.
  • Current RN license in the state of practice.
  • Current CPR certification.
  • Minimum 23 years of hospice or palliative care experience required.
  • Strong understanding of hospice regulations, clinical standards, and documentation requirements.
  • Experience with chart audits, quality improvement, or data-driven coaching preferred.
  • Ability to interpret data; Power BI experience a plus.
  • Excellent communication, teaching, and mentoring skills.
  • Strong attention to detail and documentation accuracy.
  • Ability to work independently and collaboratively across teams.
  • Valid drivers license and proof of insurance.
