Job description

Job Summary
This role will lead the development and management of strategic provider relationships that support AbsoluteCare's CKD care model. This individual will serve as the primary point of contact for nephrologists, dialysis providers, primary care practices, hospitals and skilled nursing facilities across the Ohio market. The role will support provider education, onboarding, performance management, and the success of network partnerships that enhance care coordination and outcomes for CKD patients.
This role is a key member of our value-based organization. Through our Beyond Medicine™ model, we provide comprehensive, in-home and community-based primary care, behavioral health, and social services to high-risk Medicaid and Medicare members. Our CKD Program supports patients with chronic kidney disease through proactive, personalized care that delays disease progression, reduces avoidable utilization, and improves outcomes.
Duties and Responsibilities
Provider Engagement & Education
  • Build strong relationships with nephrologists, primary care providers, and CKD-related specialists
  • Educate providers on AbsoluteCare's CKD program, workflows, value-based care model, and member engagement process
  • Host regular meetings, training sessions, and joint operating reviews to promote clinical alignment

Network Development & Maintenance
  • Identify, recruit, and introduce high-performing providers to support program growth in Ohio
  • Monitor provider satisfaction and escalate/address issues affecting collaboration and performance
  • Ensure appropriate geographic and specialty coverage for the CKD population
  • Collaborate with the Market President, Medical Director and other appropriate operational teams to establish a contractual relationship with high performing providers to align on critical value-based care activities to reduce the total cost of care

Performance Management
  • Track and report on provider performance metrics, including engagement, quality, utilization, and outcomes using internal dashboards
  • Collaborate with clinical and analytics teams to support providers in meeting performance goals
  • Assist with any other programmatic activities that align network providers to AbsoluteCare's CKD workflows

Operational Support
  • Coordinate onboarding and integration of new providers into the CKD program
  • Support accurate and timely provider directory data and issue resolution
  • Partner with internal teams (clinical, analytics, marketing operations) to ensure provider-facing materials and processes are effective
  • Utilize CRM platform or appropriate operational tool to meet provider relations goals and performance metrics/KPIs

Minimum Qualifications
Required:
  • Bachelor's degree in healthcare administration, business, or related field
  • 5+ years of experience in provider relations, network management, or provider contracting in a healthcare or managed care setting
  • Experience working with providers in value-based care, chronic condition management, or kidney care preferred. Existing relationships with key Ohio providers are preferable.
  • Familiarity with the Ohio Medicaid and Medicare landscape
  • Ability to analyze data and translate insights into actionable strategies.
  • Proven ability to build rapport, resolve conflicts diplomatically, and sustain long-term strategic partnerships.
  • High level of initiative, organization, and follow-through; capable of managing multiple priorities in a fast-paced environment.
  • Proficient with CRM systems, Microsoft Office Suite (Excel, PowerPoint, Outlook), and virtual collaboration tools (Zoom, Teams).
  • Willingness to travel regularly within Ohio (up to 50%)

Preferred:
  • Experience supporting complex care populations or risk-based programs
  • Understanding of CKD, ESRD, dialysis care coordination, and specialty referral patterns
  • Knowledge of tools like Salesforce, Tableau, or other CRM/reporting platforms
  • Ability to analyze data and translate insights into actionable strategies.

Working conditions
This job operates in the community and within a professional office environment. This role requires reliable transportation to commute back and forth between inpatient facilities and office; and routinely uses general office equipment.
