PrimaryOne Health is currently seekinga Community Health Outreach Worker for our Columbus health centers.
Essential Job Responsibilities:
  • Coordinate appointments, transportation and other social or personal services for patients as needed.
  • Assist clients in obtaining a primary care medical home; complete initial intake entry forms, maintain client records.
  • Provide community members with social support, help them connect to support groups, or assist them in completing applications for services.
  • Responsible for client follow-up broken appointments and/or incomplete service delivery.
  • Collect data to help evaluate the success of programs and identify needs for new programs.
  • Keep records of activities and write reports of their findings for Community Services Department.
  • Assess client needs; confer with medical, social service, shelter and community personnel to gain insight into client's needs; advocates on client's behalf. Establish eligibility and assist to obtain benefits and services.
  • Participates in quality improvement activities and performs other duties as assigned.
  • Moderate local travel required.
  • Attends and participates in all appropriate staff meetings, work groups, etc., as directed.
  • Other duties or special projects as assigned.

Successful candidates will have a high school diploma or equivalent; some college preferred. Community Health
Worker certification required. One to three years of relevant community service experience preferred.
