Registered Nurse (RN) Clinical Manager-Home Health

Schedule: Monday -Friday. 8:15am-5:00pm with weekend on call rotation

SUMMARY

The Clinical Manager provides oversight of all personnel and patient care services. The Clinical Manager is responsible for monitoring compliance, quality and safe delivery of services by following clinical processes. The Clinical Manager is responsible for any and all practices and duties within the scope of practice as outlined by the state. Provides backup to the Administrator/DON as needed.

KEY RESPONSIBILITIES

  • Ensure quality and safe delivery of home health services

  • Collaborates with intake, providing daily direction, managing referral volume based on staffing availability

  • Provides oversight for liaison workflow to ensure timeliness of completion as well as identify education needs for field & liaisons, ensuring workflow needs and staffing skill sets are congruent.

  • Ensure Start of Care compliance

  • Focus on Outcome Monitors as assigned

  • Implement quality improvement programs in the field

  • Participates in all liaison/administrative on-call rotation, and patient staffing as needed to manage after hour's team, staffing needs and patient related issues

  • Manages on call calendar for internal & field staff

  • Manages PTO, call offs & daily/weekly productivity review to ensure proper staffing coverage to meet the needs of the business in the field and internally

  • Completes field visits as needed

  • Participates in managing cost of good working with monitoring supply costs, overtime, staffing, bonuses and visits per episode

  • Collaborates with billing department to address authorization pro bono requests & ensures patient contact regarding authorization exceptions

  • Performs new and ongoing liaison training as defined by the Administrator

  • Manages staffing budget, at the direction of the Administrator to include: overtime & bonus oversight; reporting budget exceptions to Administrator; payroll entry of field staff bonuses; review and approve liaison timecards

  • Provides Administrator/DON with objective data related to internal and field employee performance evaluations

  • Maintains positive working relationships with referral sources including physicians, hospitals, skilled facilities, assisted livings and families

  • Assist with monthly Case Manager, Nurse and Therapy meetings

  • Reviews clinical documentation in accordance with agency protocol and Medicare/Federal guidelines

  • Process complaint workflow reporting staffing trends, neglect, abuse or theft to Administrator for resolution & follow up

  • Completes field clinician check-in reviews timely

  • Maintains active RN or PT license in state of employment

  • Participates in in-service program development

  • Consistently demonstrates core values and maintains a professional appearance as a representative of the company

  • Understands, implements and practices policies and procedures for agency operation

  • May act as back-up Administrator, as needed

  • Arrives at assigned location on scheduled work day. Works according to designated hours and on-call as needed in office and field nursing responsibilities.

  • Current applicable license as an RN or PT. Knowledgeable with nursing/therapy best practices and the scope of practice for a nurse/therapist as outlined by the state for its licensed nursing/therapy personnel.

  • Utilizes computer and phone continually to maintain constant contact with field staff, physicians, referral sources, internal staff and other departments.

  • Knowledge of ASHN billing, authorization, intake, clinical review & workflow processes.

  • Performs assigned duties with use of computer and phone.

  • Completes and submits all required documentation within specified company requirements.

  • Dexterity & vision to complete documentation on computer.

  • Other duties as assigned.

MANAGEMENT RESPONSIBILITIES

  • Responsible for the day-to-day management of field staff

  • Interviewing, hiring, training and termination of department personnel

  • Provides consistent coaching and provides challenge and opportunity to direct reports

  • Supervises direct reports with consistency and clarity of expectations

  • Completes yearly evaluations in an accurate and timely manner

  • Performs coaching, counseling and discipline as needed

  • Works with direct reports to ensure employee satisfaction and resolution of issues and concerns

  • Timecard management

QUALIFICATIONS

  • Minimum of Three (3) years of experience as an RN or PT in an acute care setting

  • Strong critical thinking skills with the ability to problem solve clinical needs

  • Ability to create positive impressions and communicate with a variety of people

  • Exemplary communication skills and customer focus

  • Ability to remain calm, have patience and be accommodating

  • Compassionate and caring while working with patients

  • Ability to resolve conflict using conflict management skills

  • Knowledgeable on nursing best practices

  • Ability to make appropriate nursing judgments

  • Ability to identify a situation and handle it with the best possible solution

  • Detail-oriented and observant.

  • Ability to resolve tough decision-making situations, providing direction to internal & field staff

  • Ability to prioritize and be timely

  • Ability to clinically triage the needs of the department to ensure team goals are met timely

  • Takes advantage of available resources (individuals, processes, departments, and tools) to complete work effectively and efficiently

  • Applies fact-based knowledge of technical skills and business understanding

  • Ability to follow directions and work as a team member

  • Valid driver's license and auto insurance.

EDUCATION AND CREDENTIALS

  • Current RN or PT license in the state of employment; RN is strongly preferred
