About the Role
As a Mobile Health Community Health Worker, you’ll connect patients to care and community resources—helping address social determinants of health and improve overall well-being. You’ll work closely with clinical teams and community partners to support patients through education, outreach, and care coordination.

What You’ll Do

  • Provide health education and outreach in community settings (homes, schools, clinics, etc.)
  • Connect patients to community resources and support services
  • Assist with care coordination, referrals, and follow-ups
  • Document services in electronic medical records
  • Support telehealth visits and care team initiatives
  • Advocate for patients and help navigate healthcare and insurance systems

What We’re Looking For

  • High School diploma required; Associate degree preferred
  • CHW certification or related experience a plus
  • Strong communication and community engagement skills
  • Valid driver’s license and reliable transportation
  • Experience with diverse or underserved populations preferred
  • Bilingual English/Spanish is required.
