ProTouch Staffing is seeking a dedicated Licensed Master Social Worker (LMSW) for a full-time contract opportunity with a leading healthcare organization in New York, NY.

The LMSW will provide psychosocial assessments, supportive counseling, discharge planning, and referral coordination for patients in an inpatient medicine/surgery setting. Shift: 9:00 AM - 5:00 PM
Hours: 35 hours per week

Key Responsibilities
Conduct comprehensive psychosocial assessments of patients and families.
Provide supportive therapy and crisis intervention as needed.
Develop and implement discharge plans to ensure continuity of care.
Coordinate referrals to community resources, healthcare providers, and support services.
Collaborate with physicians, nurses, case managers, and other healthcare professionals.
Advocate for patients and assist with addressing psychosocial barriers to treatment and recovery.
Maintain accurate and timely documentation in accordance with organizational and regulatory requirements.
Participate in interdisciplinary team meetings and care planning activities.
Required Qualifications
Master's Degree in Social Work (MSW) from an accredited program.
Current New York State Licensed Master Social Worker (LMSW) license in good standing.
Minimum 1 year of LMSW experience required.
Experience in psychosocial assessment, supportive therapy, discharge planning, and referral management.
Strong communication, organizational, and interpersonal skills.
Ability to work effectively in a fast-paced acute care environment.
Preferred Qualifications
Previous experience in an inpatient hospital setting.
Experience working within medicine/surgery units.
Familiarity with community resources and discharge planning processes.
