???? RN Home Health - Cottonwood, AZ (Local or Traveler)
Assignment Details:
13 weeks
Mon - Fri, 8am - 5pm, 40 hours per week
Contract RN must be experienced in Home Health, proficient in HCHB, and able to participate in on‑call rotation every other week to support operational and patient care needs. On-call needs are subject to change based on staff and branch needs
Productivity Units: 30 units
Qualifications: HCHB, Oasis, and HH experience required
Deliver skilled nursing care in patients' homes, following established care plans.
Complete comprehensive assessments, including OASIS documentation.
Administer medications, perform wound care, and provide patient/family education.
Collaborate with interdisciplinary teams to coordinate patient care.
Ensure timely and accurate documentation using Homecare Homebase (HCHB).
Manage admissions, recertifications, and discharges.
Participate in on-call rotation as required.
Required:
Active RN license in Georgia (or compact state).
1-2 years' experience as an RN in a clinical care setting.
2+ years of home health experience.
Proficiency with HCHB software and OASIS documentation.
Current CPR certification.
Reliable transportation, valid driver's license, and insurance.
