Join CRF's newest team - Intensive Case Management services in South San Diego County!
Intensive Case Management (ICM) is a community-based program serving adults living with serious mental illness who require a higher level of support to achieve stability and independence. The program focuses on individuals with complex needs who may be at risk of hospitalization, homelessness, or disengagement from care.
ICM utilizes a multidisciplinary, team-based approach to provide comprehensive, individualized services in the community. Staff work closely with clients to coordinate care, support access to housing and benefits, assist with medication and treatment engagement, and build skills to promote recovery and independence.
With smaller caseloads and a strong emphasis on outreach, ICM allows staff to develop meaningful relationships with clients while delivering high-quality, person-centered care. This model is designed to improve overall outcomes, reduce crisis utilization, and support long-term community integration.
CRF Behavioral Health Nurses are client-centered, compassionate, dedicated team members who make a difference in the lives of the people they serve. They embrace the opportunity to work side-by-side with their clients, helping to make lasting changes that will improve quality of life, health, and wellness. Our CRF nurses are committed to providing excellence in psychiatric nursing care.
