Job description

Crisis Stabilization and Mentoring Services

Position Title: Direct Service Provider (Crisis Stabilization)

Job Summary: The Crisis Stabilization Provider is the first line of contact with youth and families. Direct Service Providers deliver one-on-one services to youth who require support with managing their emotional, behavioral and psychological well-being. The position reports to the Program Director and Program Supervisor.

Essential Functions:

Provide, model, and teach mental health interventions.

Create and sustain relationships with youth and families.

Connect youth and families with community resources. Evaluate, manage, monitor, stabilize and support the well-being of youth and families.

Review Crisis Plans and Plans of Care.

Build partnerships with Care Coordinators and communicate weekly.

Document all interactions with youth and their families within 48 hours of contact.

Attend all required meetings.

Provide transportation for executing crisis stabilization services during crisis situations and /or

to prevent crisis situations.

Engage in initial and ongoing training.

Perform other related duties as assigned.

QUALIFICATIONS

Education:

High school diploma or equivalent.

A bachelor's degree in social work, psychology, sociology, or related field is preferred.

Experience:

Must have experience and/or educational background sufficient to meet a wide range of responsibilities. Must be enthusiastic with a cheerful outlook and possess skills and attributes that are in alignment with the core beliefs and values of Rae of Hope, LLC’s educational philosophy. Position requires excellent organization and accurate documentation of services provided. Suitable time management skills are required to ensure that youth needs are being managed properly. Candidates must be proficient in Microsoft Office and able to access web browsers for portal training, completion of daily documentation and virtual collaboration.

Physical Demands/Working Conditions:

Driving and transporting youth to community outings is a mandatory requirement for employment. Verification of Background Information Disclosure, a valid Wisconsin Driver's License, proof of auto and current auto insurance are all mandatory requirements upon hire. Reasonable accommodations can be arranged to enable individuals with disabilities to perform the essential functions of the position.
