Department: ACT-TAY (Assertive Community Treatment for Transitional Age Youth)

Employment Type: Full-time

Shift: Monday-Friday, 8am-5pm

*This position requires a willingness to travel and engage with clients outside the office setting to deliver compassionate care and support in their own environments.

Job Overview:

We are currently hiring for someone to join our team at Preferred Family Healthcare as a Registered Nurse (RN) for our Assertive Community Treatment for Transitional Age Youth (ACT-TAY) program in Hannibal, MO.

In this role, you will provide high-quality, client-centered nursing services to help youth ages 16-25 navigate their mental health journey. You will collaborate closely with our multidisciplinary team of professionals to ensure that each client receives tailored care designed to meet their unique needs and goals.

Key Responsibilities:

  • Conduct initial assessments and ongoing evaluations of clients' physical and mental health status.

  • Develop and implement individualized treatment plans that address clients' specific needs and goals.

  • Administer medications and monitor clients' responses to treatments.

  • Maintain accurate and detailed records of clients' progress, including medical histories, medication logs, and treatment plans.

  • Educate clients and their families about mental health conditions, treatment options, and self-care strategies.

  • Collaborate with other team members to coordinate comprehensive care and support for our clients.

Education and Experience Qualifications:

  • Current Registered Nurse in Missouri is Required.

  • Strong communication and interpersonal skills.

  • Familiarity with electronic medical records and relevant software applications.
