โ€‹โ€‹โ€‹
Case Manager (Home for Good)
Help Louisville turn housing goals into real homes.
Wellspring is partnering with Louisville Metroโ€™s Home for Good initiative to help move individuals experiencing chronic homelessness into permanent supportive housing. In this role, you will be part of a community-wide effort working toward housing 250 people by 2027 while providing the case management, connection, and ongoing support that helps people not only find housing, but keep it.ย  Join us and see the direct impact of your work in the lives of people across our community!
Qualifications:
  • Experience working directly with adults in a social service, mental health service, substance abuse service, and/or homeless service setting is preferred.
Education:
  • A bachelorโ€™s degree in a Human Services field such as Psychology, Sociology, Social Work, Family Studies, Human Services, or Counseling is preferred.
Salary:
  • $45,000 - $52,000 annually
Shift/Schedule: Full-time, Monday - Friday
Conditions of Employment:
  • This position requires clearances of state and federal background checks, multiple abuse/sex offender registries, and clearance of exclusion lists prior to employment.
  • Must have access to a personal, registered, and insured vehicle to transport clients (when company vehicles arenโ€™t available) and to travel between client locations and related appointments in the greater Louisville area.
  • Must possess a valid, active driver's license in good standing.
Other Requirements:
  • Home for Good (HFG) Case Managers must be prepared and comfortable working outside and in settings where our clients reside.ย
  • Must demonstrate enthusiasm for working with a disadvantaged population: individuals who are homeless, disabled by mental illness and often experiencing co-occurring substance abuse and poverty.
  • Knowledge of the theories of human behavior and the principles and techniques associated with the provision of recovery-based case management.ย
  • Strong ability to establish and maintain an effective working relationship with adults with a serious mental illness coming from homelessness, colleagues and agency/community providers/Metro/Home for Good committees.ย
  • Ability to communicate effectively and respectfully, both orally and in writing.
  • Ability to operationalize a strengths-based, person-centered approach to case management.
  • Consistently demonstrate a strong respect for the dignity of all individuals.ย
  • Working knowledge of community resources and ability to facilitate coordination activities.
Benefits That Support You (Not Just Your Job):
At Wellspring, we know you canโ€™t take care of others if youโ€™re not supported yourself. Thatโ€™s why we offer a benefits package designed to protect your health, support your life outside of work, and give you the freedom to focus on what matters most: helping people recover and thrive.
The Benefit That's Hard to Find:
One of the biggest reasons our employees stay at Wellspring is something harder to list on a benefits sheet: you can actually make and see your impact here.ย  We pride ourselves in being large enough to make a difference and small enough that your voice matters.ย  Being a mission-driven nonprofit with a collaborative culture, employees have the ability to:
  • Work autonomously
  • Advocate for clients
  • Implement ideas quickly by working closely with those who can make fast decisions
  • Focus on care rather than bureaucracy
In other words, less red tape and more meaningful work.
Health:
  • Medical insurance with employer contributions
  • Dental and vision coverage fully paid for employees (single premium only)
  • Health Reimbursement Arrangement (HRA) to help offset medical expenses
  • Flexible Spending Accounts (FSA)ย for healthcare and dependent care
  • Employee Assistance Programsย (EAP)ย for confidential support and resources
Financial Security:
  • 401(k) with 100% employer match up to 4%
  • Employer-paid basic and voluntary life insurance/AD&D for employee (employee-paid spouse and children options available)
  • Employer-paid short-term and long-term disability coverage
Time to Recharge:
  • Generous paid time off (PTO) starting after 90 days of employment
  • 9 paid holidays
Support for the Work You Do:
  • Free access toย courses that offer continuing education credits to help maintain licensure
  • Mileage reimbursement for work-related travel
  • Collaborative culture where staff ideas are heard and can quickly translate into improvements
  • Free downtown parking
  • Use of company vehicle
