Job description

Job Title: Registered Nurse Case Manager (RN Case Manager)
Location: Jeffersonville, IN 47130 (field-based; covers Clark, Floyd, Harrison, Crawford, Washington, Orange + as needed)


Schedule: Monday–Friday, 8:00 AM–5:00 PM + rotating weekend on-call


Pay: $56.14/hr

Role Overview:


Hiring a Registered Nurse Case Manager to provide field-based care management and home health support. The RN Case Manager completes comprehensive assessments, builds individualized care plans, coordinates services/resources, and supports safe transitions of care for medically complex, high-risk patients.

Key Duties:

  • In-home/community assessments for high-risk patients
  • Care plan development, ongoing reassessment, and clinical documentation
  • Coordinate transitions of care with providers, social workers, discharge planners, and community resources
  • Provide patient education and ensure treatment plan compliance
  • Clinical skills support as needed: wound care, IV therapy, drains, G-tubes
  • Track outcomes and identify prevention/health promotion opportunities
  • Rotating weekend on-call coverage

Required Qualifications:

  • Active, unrestricted IN RN license (compact accepted)
  • Nursing degree (Associate or Bachelor’s)
  • Strong independent field work skills + accurate documentation
  • Home health / case management experience preferred
  • HCHB/WellSky (or similar) experience preferred
  • Valid driver’s license; able to travel locally between visits
