The Case Manager MAT provides comprehensive direct and indirect case management for Substance Use Disorder (SUD) services to inmates at assigned detention centers. Responsibilities include coordinating treatment plans, conducting educational groups, and ensuring continuity of care post-release. The role requires close collaboration with medical, security, and administrative staff to deliver integrated, effective support for inmates undergoing MAT.


  • Assists the Director of Recovery and Reentry Services (DRRS) in managing the case management needs of individuals in MAT treatment, ensuring coordinated services integrated with security, medical, and administration.
  • Conducts educational groups, provides routine case management services, and ensures appropriate documentation and active involvement in treatment team meetings.
  • Provides discharge planning services to support MAT continuation post-release, ensuring all services are documented in the medical record as per protocol.
  • Attends clinical supervision, required trainings, and relevant meetings, assists the DRRS with data collection and entry for all MAT visits, and prepares statistics for third-party agencies.
  • Networks with administration, correctional officers, medical, and outside providers to maintain positive working relationships, abides by security regulations, protects the confidentiality of inmate information, and adheres to HIPAA Regulations.

Education

  • Bachelor’s degree in substance use, social work, psychology, or related field from an accredited college or university or nursing license; LPN

Experience

  • One (1) year experience required.
  • Mental Health, Substance Use Disorders, Medication Assisted Treatment, or Case Management in a corrections environment preferred.

Licenses/Certifications

  • Possess and maintains current CPR certification.
