Wellness Nurse - Franciacan Vilage - Lemont, IL

Position Summary

The Wellness Nurse is responsible for promoting the health, safety, and well-being of residents by providing clinical oversight, health assessments, medication management, and care coordination. Working collaboratively with residents, families, physicians, and interdisciplinary team members, the Wellness Nurse ensures that residents receive individualized care that supports their independence, dignity, and quality of life while maintaining compliance with all applicable regulations and community policies.

Essential Functions, Key Duties, & Responsibilities

  • Conduct comprehensive health assessments for prospective and current residents.
  • Develop, implement, and update individualized service plans based on resident needs and physician recommendations.
  • Monitor residents for changes in physical, cognitive, and emotional health and initiate appropriate interventions.
  • Perform nursing assessments following falls, illness, hospitalization, or significant changes in condition.
  • Coordinate physician appointments, laboratory services, therapy, hospice, home health, and other healthcare services.
  • Maintain accurate, timely, and confidential clinical documentation.
  • Provide clinical guidance and support to clinical associates at the community.
  • Respond to resident emergencies and provide appropriate nursing interventions.
  • Ensure compliance with resident rights, confidentiality, and professional nursing standards.
  • Communicate resident health concerns and changes in condition to physicians, families, and responsible parties in a timely manner.
  • Participate in resident care conferences and interdisciplinary meetings.
  • Educate residents and families regarding health conditions, medications, and available healthcare services.
  • Build positive relationships that promote trust, compassion, and resident-centered care.
  • Maintain compliance with all federal, state, and local assisted living regulations.
  • Assist during state surveys and regulatory inspections.
  • Ensure required nursing documentation is complete, accurate, and survey-ready.
  • Maintain resident records in accordance with HIPAA and community policies.
  • Perform other duties as assigned.
  • Comply with all policies and procedures and any updates.

Position Requirements

Education: Graduate of an accredited nursing program

Licenses/Certifications: Current Licensed Practical Nurse (LPN) or Registered Nurse (RN) license required

Experience: 1+ years of experience in a clinical/nursing setting within the senior living industry required; 1+ years of supervisory experience preferred

Skills & Abilities: Outstanding customer services and bedside manner
