Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Clinical Manager (RN) for a growing home health organization in San Antonio, Texas.
POSITION OVERVIEW
Hybrid Clinical Manager leadership role supporting home health operations
Coordinate patient care planning, referral reviews, and interdisciplinary case management
Review new patient referrals for clinical appropriateness and approve/co-sign clinical orders
Provide guidance to field clinicians regarding OASIS documentation and utilization review (No OASIS Review Caseload Required)
Partner with physicians, therapists, schedulers, QAPI, and field staff to ensure regulatory compliance and continuity of care
Assist the Director of Nursing with clinical oversight, scheduling support, and call rotation responsibilities

Requirements

Active Registered Nurse (RN) License REQUIRED
Minimum One Year Recent Registered Nurse Experience REQUIRED
Minimum One Year Home Health Experience REQUIRED
Thorough Knowledge of Medicare Conditions of Participation (CoPs) REQUIRED
OASIS Documentation Knowledge REQUIRED
Homecare Homebase (HCHB) Experience REQUIRED
Valid Driver's License REQUIRED
Completion of Candidate Attestation & Screening REQUIRED
Leadership or Supervisory Experience Preferred
Utilization Review and Quality Improvement Experience Preferred
