Summary:

The Wellness Nurse (RN) is responsible for providing high-quality nursing care, conducting resident assessments, coordinating clinical services, and supporting the overall health and wellness of residents. Working closely with the Director of Nursing Services and interdisciplinary team, the Wellness Nurse ensures individualized care plans are implemented while maintaining compliance with all federal, state, and local regulations.

Duties and Responsibilities:
  • Conduct resident assessments upon admission and perform ongoing nursing evaluations to identify changes in condition and care needs.
  • Develop, implement, and update individualized resident care plans in collaboration with physicians, families, and the interdisciplinary care team.
  • Provide direct nursing care and oversee resident wellness in accordance with physician orders and community policies.
  • Monitor resident conditions, communicate changes to physicians and families, and ensure timely follow-up on physician orders.
  • Coordinate services with pharmacy, hospice, home health, rehabilitation providers, and other healthcare partners.
  • Perform medication and treatment audits to ensure accuracy and regulatory compliance.
  • Participate in Health Maintenance Meetings and interdisciplinary care conferences.
  • Maintain accurate clinical documentation, nursing notes, assessments, and resident records.
  • Support infection prevention initiatives, skin integrity monitoring, and quality assurance programs.
  • Assist with resident admissions, transfers, discharges, and emergency response procedures.
  • Provide clinical guidance and support to nursing staff and caregivers while promoting exceptional resident-centered care.
  • Attend required training courses and participate in professional development activities.
Knowledge and Skills:
  • Excellent verbal and written communication skills.
  • Strong clinical assessment, critical thinking, and problem-solving abilities.
  • Ability to work effectively within a collaborative team environment.
  • Excellent organizational and time management skills.
  • Strong knowledge of geriatric nursing, medication management, and resident-centered care.
  • Proficient in Microsoft Office, electronic medical records, and clinical documentation systems.
Qualifications:
  • Current Registered Nurse (RN) license in good standing in the Commonwealth of Virginia.
  • Minimum of 2 years of nursing experience; senior living, assisted living, memory care, or long-term care experience preferred.
  • Bachelor of Science in Nursing (BSN) preferred.
  • Ability to clearly communicate with residents, families, physicians, and team members while maintaining a compassionate and professional demeanor.
  • Must be able to read, write, and follow verbal and written instructions in English.
  • Must maintain a clean and professional appearance in accordance with community standards.
  • Ability to work flexible hours, including weekends, evenings, and holidays as needed.
  • Must complete required health screenings, including TB clearance.
  • Must successfully pass all required Virginia criminal background checks.
Mental and Physical Requirements:
  • Requires the ability to stand, walk, bend, and move throughout the community for extended periods.
  • Must be able to lift and/or assist with items or equipment weighing up to 25 pounds.
  • Requires reaching above, below, and at shoulder height.
  • May be required to assist residents during emergency situations or evacuations.
  • Ability to remain calm, compassionate, and positive while working in a fast-paced clinical environment.
