Hospice Registered Nurse PRN (After Hours)

Overview
The Care Team is seeking a Registered Nurse to join our growing team. The After Hours RN provides skilled nursing care and compassionate end-of-life support to hospice patients in their homes or facility settings, working collaboratively with the interdisciplinary team.
Schedule, Hours & Territory
• This role is scheduled for after-hours coverage beginning at 4:30 PM, with occasional weekend support as needed.

Key Responsibilities
• Conduct comprehensive patient assessments and develop individualized plans of care
• Provide skilled nursing care in accordance with hospice standards and physician orders
• Coordinate care with physicians, caregivers, and interdisciplinary team members
• Educate patients and families on symptom management, disease progression, and end-of-life care
• Document assessments, interventions, and care plans accurately and in a timely manner in the EMR HCHB (Homecare Homebase)
• Participate in interdisciplinary team meetings and care planning
• Support after-hours patient needs as part of an on-call rotation when applicable
• Active Registered Nurse (RN) license
• Hospice or home health experience preferred
• Strong clinical assessment and care coordination skills
• Ability to work independently in the field
• Strong communication and documentation skills
• Experience using HCHB (Homecare Homebase) preferred
Field Role Requirements
• Active driver's license
• Ability to pass a criminal background check
• Ability to pass a Motor Vehicle Record (MVR) check
• Current TB testing in accordance with agency policy
• Current CPR certification as required for the role
