Job description

Are you a compassionate and dedicated nurse looking for a versatile opportunity to deliver exceptional care? We currently have opportunities for both part-time and full-time weekend positions, providing flexible options to accommodate different scheduling preferences for rotating weekends and weekdays.

Registered Nurse is responsible for providing direct patient care as needed, with appropriate follow up to visits and documentation of care for agency clients in accordance with Quality Standards and under the guidance of the Weekend Clinical Manager. Participates in offices duties as required such as scheduling, coordination of care, and referral processing. Collaborates with staff to ensure the bridging of information from the weekend to weekday to facilitate appropriate management of patient care issues. Contributes to clinical operational process and development. Ensures adequate skills and adherence with agency policies and procedures. Assists with On-call as needed.

Performance Responsibilities and Standards:

1. Ensure quality and safe delivery of home health services.

  • Registered Nurse is able provide care to a diverse set of clients.
  • Follow plan of care based on client assessment and OASIS evaluation.
  • Ensure that all documentation meets quality standards:
  • Completes all OASIS evaluations.
  • Documentation reflects nursing care and interactions.
  • Documentation is clear and concise.
  • Documentation is completed in timely manner
  • Understand and follow nurse code of ethics and standards of practice.
  • Coordinate with internal and external resources to meet patient needs.
  • Maintain established productivity expectation
  • Communicate client care issues to Case Manager/Clinical Supervisor.

2. Comply with Nascentia Health policies and procedures and quality initiatives.

  • Maintain confidentiality.
  • Demonstrate knowledge and use of infection control practices.
  • Demonstrate knowledge of federal, state and local government rules and regulations.
  • Demonstrate knowledge and compliance with agency policies and procedures.
  • Adhere to quality improvement standards.
  • Keep personnel requirements up-to-date; i.e., health requirements, licensure requirements, and other Nascentia Health requirements.
  • Demonstrate skills competency through periodic evaluations.

3. Participate in personal growth and development.

  • Participate in educational in-services and meetings.
  • Maintain and update clinical knowledge on an ongoing basis.

4. Promote continuity of care for referred clients

  • Evaluate and process referrals from hospitals, physician offices, skilled nursing facilities, community agencies, patients and their families as needed to assist the weekend supervisor.
  • Answer inquiry calls from clients, families, physician offices, community agencies and insurance companies.
  • Update referral information as appropriate
  • Takes verbal orders from referrals sources.

5. Promote quality assurance as it relates to home care planning

  • Participate in quality improvement activities related to home care planning and coordination.
  • Adhere to quality standards including the confidentiality of client information.
  • Runs and utilizes capacity information to manage the number of referrals accepted each day.
  • Maintains an organized report of all referrals coming in to the agency from hospitals, physician offices and nursing home facilities.
  • Oversee the review of clinical information, orders and other communications to assure referral information is complete and accurate information to field staff

6. Assist with any weekend scheduling needs.

7. Perform all other duties as assigned.
Requirements:
1. Current New York State registered professional nurse license (degree must be from an accredited school of nursing). BSN a plus.
2. Current NY State driver’s license, automobile insurance and a vehicle which can be used for work
3. Weekend schedule (8am-4:30pm) for every Saturday and Sunday preferred; rotating weekends available- minimum two weekends a month required. An in office position on weekends, with field-based visits required. (FT, PT, Limited schedules available)

Physical Requirements:

1. Speaking/visual/hearing ability sufficient to comprehend and express written/verbal communication.

2. Ability to perform tasks involving physical activity including:

  • Pushing/pulling – typical weight 50 pounds
  • Maximum weight 150 pounds
  • Lifting/carrying – typical weight 25 pounds
  • Maximum weight 40 pounds

3. Ability to walk and drive in varied weather conditions throughout the year.

4. Ability to use telephone/IT equipment to communicate/document.

5. Bloodborne Pathogens Exposure Category I – Employee has tasks that routinely involve exposure or potential exposure to blood, body fluids or tissues.
