Position Overview

Healing Arts Social Work Services is seeking a dedicated and licensed Master Social Worker to join our growing team. This full-time, remote position focuses specifically on providing vital support to seniors in school settings, where you will assess, advocate for, and enhance the quality of life for our clients and their families.

Work Environment

This role offers the flexibility of a remote work setting while primarily focusing on the needs of seniors within a school facility. The work hours are Monday to Friday from 8:00 AM to 4:30 PM, with occasional evening or weekend hours based on client needs. As such, adaptability in your schedule will be essential to accommodate client visits, ongoing care, and case management efforts. Moreover, on-call availability may be required for urgent situations that necessitate immediate social support.

Key Responsibilities
  • Conduct in-depth assessments of seniors' psychosocial needs through interviews and observations, evaluating emotional, mental, and social factors that affect their everyday lives.
  • Collaborate closely with clients and their families to craft personalized care plans that prioritize their specific goals, preferences, and any required interventions.
  • Provide direct counseling and emotional support, helping seniors navigate significant life transitions such as loss, health changes, or new environments.
  • Act as an advocate for clients to ensure their rights are respected and they have access to necessary community resources, including healthcare services and support groups.
  • Work as part of an interdisciplinary team, coordinating with nurses, caregivers, and other healthcare professionals to enhance comprehensive care for seniors.
  • Perform regular home visits to monitor clientsโ€™ well-being, assess their living conditions, and proactively address any concerns related to safety or health.
  • Facilitate effective communication among family members involved in the senior client's care to reduce conflicts and enhance support structures.
  • Educate clients and their families about available community resources and programs that can enrich their social engagement and overall quality of life.
  • Maintain thorough documentation of client assessments, care plans, progress notes, and relevant interactions, ensuring compliance with ethical standards and regulatory requirements.
  • Keep abreast of current developments in social work practices, policies, and regulations to uphold the highest level of professional care and support.
Patient Population / Client Focus

This role specifically focuses on the senior population within school facilities, emphasizing the social, emotional, and psychological aspects of aging. You will engage with clients who may be dealing with various challenges related to their health, family dynamics, and community resources. Qualifications
  • Possession of a Master's degree in Social Work from an accredited institution.
  • A current and valid license as a Master Social Worker (LMSW) in the state of Pennsylvania.
  • A minimum of two years of relevant experience in social work, preferably within community-based or geriatric settings.
  • Deep understanding of social work principles, aging-related matters, and knowledge of local community resources.
