Connected Cardiovascular Care Associates (C3) is seeking an experienced Nurse Practitioner or Physician Assistant to join our growing cardiology practice serving the Heath and Dallas communities. This role offers a balanced mix of outpatient clinic care and light inpatient rounding, within a collaborative, physician-led environment.

C3 is an innovative, physician-led cardiovascular practice founded by Dr. Tony Das, a nationally recognized interventional cardiologist. Our practice delivers comprehensive cardiovascular care across coronary, vascular, structural heart, electrophysiology, heart failure, and preventive cardiology.

Position Overview
  • Schedule: Monday-Thursday 8:30 AM - 5:00 PM, Friday 8:30 AM - 3:00 PM
  • Locations: Weekly rotation between Dallas and Heath clinics
  • Hospital Rounding: Light inpatient rounding across Dallas, Plano, Rockwall, and Rowlett
  • Call: 1:4 weekend rotation
  • EMR: eClinicalWorks (outpatient) and Epic (inpatient)

This is primarily an outpatient role with a manageable inpatient component and strong clinical support.
Key Responsibilities
  • Evaluate, diagnose, and manage cardiology patients in the outpatient setting
  • Perform and document history, physical exams, and follow-up care in the EMR
  • Review and communicate diagnostic testing results, including imaging
  • Collaborate with physicians, APPs, and clinical staff on patient care plans
  • Triage patient needs, including messaging, pre-operative clearance, and peer-to-peer reviews
  • Provide oversight for in-office testing (CT, PET CT, nuclear stress testing)

Qualifications
  • NP or PA from an accredited program
  • Nurse Practitioners: AGACNP required
  • Minimum 5 years of cardiology experience required
  • Active Texas license in good standing
  • DEA and Controlled Substance Registration
  • ACLS and BLS certification
