Summary

Primary Care Advanced Practice Clinician Opportunity with Samaritan Health Services 

Samaritan Health Services, a non-profit integrated healthcare delivery system, is seeking a physician assistant or nurse practitioner to join our Family Medicine team at our Park Street Clinic in Lebanon, Oregon. Our Park Street Clinic is an established certified Rural Health Clinic with experienced clinicians who are passionate about building healthier communities within a certified Patient-Centered Medical Home model of care.

Job Specifics

  • 1.0 FTE (34 hours per week)
  • 4.5 days per week is full-time
  • Up to 20 patients per day
  • Call schedule: phone call only with 1-2 days a month plus one weekend every 2-3 months
  • Outpatient Family Medicine — a mixture of preventative and acute visits

Job Requirements

  • Excellent clinical skills, communication skills, and a strong commitment to providing excellent care that is team oriented.
  • Eligibility to work in the United States
  • Graduate of an accredited Physician Assistant program/Nurse Practitioner program
  • Licensed in the State of Oregon
  • Experience required.
