Hospice Registered Nurse (RN) Case Manager – Full-Time

We are seeking a compassionate and experienced Hospice RN Case Manager to provide patient-centered, end-of-life care.

Key Responsibilities
  • Manage a dedicated hospice caseload as the primary RN Case Manager
  • Perform patient assessments, admissions, and individualized care planning
  • Deliver skilled nursing care focused on comfort and dignity
  • Coordinate care with physicians and interdisciplinary team members
  • Conduct routine visits and adjust care plans as needed
  • Provide end-of-life support, including family education and pronouncement
  • Maintain timely, accurate documentation
  • Supervise Home Health Aides in compliance with regulations
Qualifications
  • Graduate of an accredited RN program
  • Active Illinois RN license
  • Current CPR and First Aid certification
  • Hospice or home health experience preferred
