Job description


Primary Duties And Responsibilities
  • The Chaplain provides and coordinates the delivery of pastoral care and spiritual support to patients, their families, and staff during events of hospitalization, crisis, bereavement, or when facing end-of-life questions or decisions.
  • The Chaplain models and collaborates with the hospital's interdisciplinary teams in respecting and providing spiritually and culturally competent patient-centered care.
  • Understands the full range of individual beliefs, customs, traditions, and practices represented in the community, and serves as a liaison between the hospital and patient/family's community of faith.
  • Maintains accurate documentation of patient visitations in the electronic health record.
  • Develops expert-level competencies that can be drawn upon to provide education and development opportunities for their colleagues and interdisciplinary teams.
  • Develops knowledge of assigned areas and works within the scope of service.
  • Develops skill sets and practice standards to meet the needs of the patients/families and health care team for their areas.
  • Available outside normal business hours for emergency response on-call coverage during weekdays and/or on weekends.
  • Performs other duties as assigned.

Hospice Chaplain
  • The Chaplain provides and coordinates the delivery of spiritual and emotional support to patients, their families, caregivers, and staff.
  • Supports and guides patients, their families, caregivers, and staff to assist them through periods of transition, grief, loss, and anxiety.
  • Provides spiritual interventions at times of crisis or death.
  • Guides patients' families and caregivers through spiritual and existential questions and concerns regarding death.
  • Provides comfort and solace during the end-of-life experience.
  • The Chaplain must always be appreciative of and respectful toward the full range of individual beliefs, customs, traditions, and practices represented in the community, and serve as a liaison between the hospice and patient/family's community of faith.
  • Maintains accurate documentation of patient visitations.
  • Available outside normal business hours for emergency response on-call.
  • Performs other duties as assigned.

Qualifications
Education and Experience
  • Master of Divinity degree or equivalent from an accredited theological/faith-based institution. Four Units of Clinical Pastoral Education in a Hospital setting. Eligible for Board Certification by a recognized accredited organization within one year of start date.
  • Ecclesiastical endorsement required.
  • Experience in pastoral care and chaplaincy in a healthcare setting preferred.
  • Board Certified Chaplain in good standing with a recognized and accredited organization preferred.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
  • Hospice Chaplain: Master of Divinity degree or equivalent from an accredited theological/faith-based institution. Minimum of two units of Clinical Pastoral Education. Experience working with death and dying, individuals/family/caregiver.
