Specializing in respiratory and chronic disease management, we are committed to delivering compliant, high-quality, patient-centered care.
Essential Duties:
Under the general supervision of the Branch Manager, the Hospital Care Coordinator is responsible for coordinating durable medical equipment (DME) and respiratory services. This role is strictly non-sales in nature and focuses on operational coordination, documentation accuracy, and regulatory compliance. The Hospital Care Coordinator does not provide education, instruction, or services to a patient prior to receipt and verification of a valid physician order; and may conduct limited administrative coordination only as necessary to facilitate order processing in compliance with company policy.
Responsibilities:
  • Serve as the primary point of contact between SuperCare Health and hospital personnel for coordination of ordered equipment.
  • Ensure all required documentation and physician orders are received, complete, and accurate and patient eligibility is verified prior to engaging with patient or authorized family member/care giver.
  • Coordinate delivery of equipment to hospital rooms or patient homes in collaboration with internal teams and in a manner that avoids duplication or conflict with other providers involved in the patient's care.
  • Maintain accurate records of referrals, orders, and communications.
  • Provide status updates to hospital staff regarding equipment delivery timelines.
  • Maintain inventory levels in assigned hospital convenience closets.
  • Collaborate with Customer Care and Dispatch teams as needed to ensure efficient service delivery.
  • Attend all department meetings and training sessions as scheduled.
  • Maintain clear separation from hospital staff roles and responsibilities; employee acts solely on behalf of SuperCare Health and not the facility.
  • Perform all other duties as assigned by department leadership.
Compliance and Regulatory Requirements:
  • Strictly adhere to all applicable federal and state laws, including but not limited to the Anti-Kickback Statute (AKS), Stark Law, and False Claims Act (FCA).
  • Do not engage in any activity that could be interpreted as offering or providing remuneration, inducements, or incentives in exchange for patient referrals.
  • No sales, marketing, or referral-generation activities are permitted as part of this role.
  • Employee compensation shall not be based on, tied to, or influenced by the volume or value of referrals or business generated.
  • This role is not subject to quotas, commissions, or performance metrics based on referral generation or revenue.
  • Interactions with healthcare providers must remain strictly informational and operational, without attempting to influence referral decisions.
  • Report any suspected compliance concerns immediately.
  • Present all patients with their right to freely choose a DME supplier and document such choice prior to coordinating services.
Physical Demands:
  • Must be able to walk long distances across large healthcare facilities.
  • Must be able to lift and carry equipment up to 75 lbs.
Work Environment:
  • Fast-paced healthcare facilities with emphasis on coordination and compliance.
  • Requires managing multiple discharges and tight deadlines.
  • High-level communication with healthcare professionals.
Work Experience Requirements:
