Job description


Primary Duties and Responsibilities
The Outpatient Care Coordinator Lead assists with scheduling tests and treatments, including the coordination of multiple appointments amongst several different departments, programs and specialties. The responsibilities of this position include insurance verification, assisting with the pre-authorization process, and acting as a liaison between the pre-authorization team, physician office and patient. The Care Coordinator provides updates to the patient and doctors on timing and status of the authorizations as delays or insurance denying a pre-authorization could have an impact on the patient's health and/or testing and treatment options. Once the pre-authorization is complete and the appointments scheduled, the Care Coordinator provides the patient with an itinerary when possible. Assists with referrals as requested. Links family/patient to resources as needed. Performs other duties as assigned.
The Outpatient Care Coordinator Lead is responsible for daily operations of the Outpatient Care Coordinating team and for all training required of the Outpatient Care Coordinator in Care Coordination. Coordinate, leads, supervises, monitor and controls the human and physical resources of the Outpatient Care Coordinator team. Provides, maintains and update training manuals and resource tools for care coordination. Monitors quality in customer service, scheduling of appointments and pre-registration of accounts. Works with all staff in the achievement of Departmental and Organization goals.
GAMMA KNIFE DEPT (GK): In addition to the above: Attends Tumor Boards to identify physician recommended GK treatment patients. Ensures GK equipment and supplies are onsite and available for use.
Interventional Radiology Dept: Provides daily support and education to IR scheduling specialist team at Red Hill location. Floats to hospitals to support and cover Lead Clerks assisting IR Nursing team. Provides scheduling team support as front line scheduler to assist with incoming calls, authorizations, policy updates, auditing, managing daily reports and staff assignments and schedules, communication and documentation individually and as a team, when errors occur. Liaison between Hoag Imaging Locations as a point of contact for assistance with Benefactors, Clinics, and patient relations, etc.
Qualifications
Education and Experience Required:
  • High School diploma or equivalent
  • Minimum 5 years hospital or outpatient acute healthcare experience
  • Must possess strong knowledge in hospital scheduling, registration and insurance verification processes
  • Interventional Radiology Only: Must have 1 year Imaging Scheduling Specialist Lead experience at Hoag

Preferred:
  • Knowledge and understanding of patients benefits, plan coverage and claim responsibility.
  • GAMMA KNIFE: A designated EPIC Superuser is preferred.
