Job description

Licensed Practical Nurse

Location: New Lisbon, Wisconsin
Pay Rate: $33.00/hr – $36.00/hr
Contract: 13 Weeks
Schedule: PM Shift, 2:00 PM – 10:00 PM, Every Other Weekend
Guaranteed Hours: 36 hours/week
Experience: LTC experience preferred
EMR: Meditech

About the Role
We are seeking a Licensed Practical Nurse for a long-term care contract opportunity in New Lisbon, WI. This role supports resident care, medication administration, condition monitoring, family updates, and collaboration with CNAs and nursing leadership.

Responsibilities

  • Administer medications while following the 5 Rights

  • Monitor residents and report changes in condition

  • Notify physicians, family members, and charge nurse as needed

  • Assist CNAs with call lights and resident support

  • Report any concerns related to abuse, neglect, or misappropriation

  • Maintain accurate documentation in Meditech

  • Perform other nursing duties as assigned

Requirements

  • Active Wisconsin LPN License required

  • Current CPR/BLS certification required

  • LTC or related nursing experience preferred

  • Strong attention to detail required

  • Must be able to work PM shift and every other weekend

  • Holiday coverage required
