Summary:
Summary
The RISE (Refugee Integration and Support Efforts) Case Manager works directly with Haitian refugees in Kent and Sussex counties, providing direct case management services, group support, and employment services with a trauma-informed service delivery focus. The RISE Case Manager will be instrumental in cultivating connections and ensuring access to therapeutic services and community resources. Working with a team of RISE Case Managers and community agencies, you will help ensure comprehensive integration for refugee families in Delaware.
Responsibilities:
  • Provide case management for assigned refugee clients in Kent and Sussex counties.
  • Deliver workforce/outreach services, trauma-focused case management, and assistance running support groups.
  • Document case notes and interventions in state database systems and JFS' electronic health record, including InSync, Assist Worker Web (AWW), Apricot, and IRIS
  • Provide outreach and engagement to local community partners and potential refugee referral sources
  • Provide transportation assistance to social services, DMV, medical screenings, and other appointments
  • Link clients with employment and training services.
  • Other duties as assigned.

Requirements:
  • Bachelor's degree in social services or social work from an accredited school with prior experience working with refugee, immigrant populations; master's degree preferred
  • Must speak Haitian Creole
  • Experience working with various client populations, Haitian Creole in particular.
  • Experience using Microsoft Office applications, e.g., Outlook, Word, Excel, Team
  • Valid driver's license required
  • Problem solver with good analytical skills, strong communication and language skills, case management capabilities, ability to effectively manage time and multiple client demands with experience de-escalating and managing crisis situations.

Pay Range: $42,000 - $45,000 per year
