Job description

Position Summary
Theย Coordinator of Person-Centered Services positionย (Workforce Development Department) willย provide case coordination and advocacy primarily forย individuals with disabilities who are employed and supported byย OBI. This position will help individuals and families navigate the โ€œpathwayโ€ toย services andย is a critical member of an individualโ€™s Person-Centered Planning Team. This Case Coordinator willย have extensive knowledge of OBIโ€™s funding and referral sources (i.e., DDA, DORS, CCSโ€™s, schools, etc.) and will establish and maintain excellent professional relationships with all stakeholders. The salary for this position is $50K - $58K.
Essential Functions & Responsibilities
  • Provide case coordination services toย the participantsย in OBI programmingย byย ensuringย meaningful services and the highest quality of care and support through theย person-centeredย planning (PCP) process.ย 
  • Work collaboratively with Coordinators of Community Services (CCSs) to facilitate timely Person-Centeredย Planningย (PCP)ย meetings.ย 
  • Work with each participant and their team to discuss and establish person-centered goals to be included in the Service Implementation Plan (SIP).ย ย 
  • Maintain close communication with the OBI Workforce Development team (Employment Specialists, Career Coaches and Manager of Workforce Development) and/or direct support professionals to assist with program planning and monitor the implementation and progress toward PCP goals.ย 
  • Manage app-based transportation services such as Uber or Lyft, including but not limited to arranging rides.
  • Ensure participants do not go very budget this included but is not limited to ensuring individuals to not exceed their transpiration budget.
  • Maintain up-to-date knowledge of DORS authorizations and regulations.ย 
  • Initiate DORS applications.ย 
  • Develop and submit required documentation to request and secure appropriate DDA funding for each participant.ย 
  • Monitorย participantย documentation for accuracy, quality, and to remainย up to dateย on each participantโ€™s progress.ย 
  • Involve OBIโ€™s Workforce Development team whenever a participant is interested in employment.ย 
  • Involve OBIโ€™s Manager of Personal Supports whenever a participant is interested in Personal Supports services from OBI.ย 
  • Complete the attendance process as determined by OBI to ensure accurate billing and service delivery.ย  ย ย 
  • Review and approve required documentation on time, including but not limited to OBIโ€™s online database platformย service logs.
  • Review PCPโ€™s on LTSS for accuracy and provide feedback to CCS and/or Manager of Person-Centered Services regarding any necessary changes that need to be made before services can be accepted.
  • When requesting/advocating for individual services, provide DDA and/or the CCS with relevant and substantial back-up documentation and data to justify the request.
  • Assure Behavior Plan compliance by working with the Behavior Specialist and Sr. Manager of Quality Assurance.ย 
  • Assure nursing and medication administration compliance by working with the Sr. Manager of Nursing and Quality Care and the Sr. Manager of Quality Assurance.ย 
  • Train DSPs on required individual specific training and maintain required documentation thatย the training occurred.ย ย ย 
    • Example: Specialized protocols for feeding or toileting,ย documented in the PCP.ย 
  • Maintain accurate and complete records for each participant as instructed inย the client management system.ย 
  • Assist to assure quality services and agency regulatory compliance.ย 
  • Follow allย DDA and OBIย regulations when investigating, monitoring, and reporting incidents.ย 
  • Provide back-up transportation as needed for participants.ย 
  • Provide Direct Care as needed for participants.
  • Communicate with families/caregivers, andย CCSsย on a regular basis, being sure to communicate any changes or important information with the entire team.ย 
  • Attend regularย departmentalย meetings.ย 
  • Intervene in crisis situations to assistย participantsย in resolving time-sensitive issues; as well as ensuring the appropriate services are received.ย 
  • Establishย relationships with other appropriate community nonprofits and service providers, to collaborate and share resources.ย 
  • Maintain up-to-date knowledge of DDA funding and service definition.ย  ย ย 
  • Maintain compliance with all OBI, DDA, OHCQ, and DORS policies and regulations at all times.ย 
  • Submit all OBI forms to the CCS as part of the PCP in accordance with DDA policies and regulations.ย 
  • Provide assistance with virtual supports as needed.ย  Including but not limited to the AM and PM Zooms.ย 

Non-Essential Dutiesย 
  • Attend external meetings, conferences and trainings asย requested by supervisor.ย

Qualificationsย 
  • Minimum ofย 1ย year of successful experience working with individuals who have disabilitiesย or providing case coordination/case management.ย 
  • Experience with workforce development/employment services (vocational rehabilitation, DORS, DDA employment services etc.)
  • Experience in using a client management system; I Care Manager preferred.
  • Experience in LTSS and a knowledge of the DDA service definitions preferred.ย 
ย 
  • High school diploma or equivalent required; Bachelorโ€™s degree in a related field preferred.ย 
  • Excellent written and oral communication skills.
  • Highly organized and demonstrated ability to work well independently and as part of a team.ย 
  • Valid Driverโ€™s license, no more than 3 points on driving record and 3+ years driving experience.ย 
  • Ability to perform and meet job and physical requirements listed below.
  • Proficient in Microsoft Office and a broad understanding of (or ability to learn about) technology that supports community development and person-centeredness and/or virtual service delivery, including apps such as Zoom and Google Voice.ย
