Major Responsibilities

  • Patient Transfer Coordination: Facilitate safe and timely transfers of patients with emergent, urgent, or medically complex conditions; collaborate with physicians, transport staff, and other healthcare providers to ensure appropriate patient placement.

  • Clinical Assessment: Perform triage assessments to determine the appropriate level of care and subspecialty needed for transferring patients; utilize clinical knowledge to gather and review patient information, ensuring all necessary data is collected for transfer decisions.

  • Communication: Utilizing closed-loop communication act as a liaison between referring and receiving facilities, facilitating verbal communication and documentation of transfer requests; coordinate with multiple clinicians to discuss patient cases and determine the best course of action; provide exceptional customer service to patients, families, and external healthcare providers.

  • Interprofessional Collaboration: Collaborate effectively with interdisciplinary teams to deliver patient-centered care, ensuring shared decision-making, and optimal outcomes; determination of when to use the chain of escalation/resolution when appropriate.

  • Documentation: Document all transfer activities in electronic medical record (EMR) and transfer center software; prepare and review monthly reports on transfer center activities and trends.

  • Quality & Safety Improvement: Participate in quality improvement activities to enhance transfer center processes and patient outcomes; monitor and report trends related to patient transfers; participate in debriefing sessions as necessary to address adverse events;
