o   Home Health Services – York County

o   Address: 226 Northpark Dr., Rock Hill, SC, 29730

●        General Information:

o   Average patients/day? 8/day minimum – 9 points must be met per day

o   Minimum Years of Experience? At least 1 year of Home Health experience

o   Will you accept a first-time traveler? No

●        Patient Types/Common Diagnoses:

o   Adults – only

▪          Chronic meds, Cardiac, Heart Failure, COPD, Wounds, Drains

●        Required Licensure & Certifications:

o   BLS, valid DL and insurance

●        Skills required:

o   Home Health experience, Homecare Homebase charting experience is required; OASIS-E (assessment tool) experience

o   Patient assessments, experience with wound care, pleurx, infusion, venipuncture, case management, SOC (Start of Care) Therapy (comfortability to conduct initial patient visits independently, chronic disease management/maintenance, plan of care, medication management, patient/family teaching

●        Support within the Department:

o   RNs

o   Triage – functions as weekend leadership

o   EMR: Homecare Homebase – experience required
