  • Join an established and well-respected team of 8 physicians and 3 APP's
  • Hybrid role
  • This position will combine virtual and in-person work, with focus on flexibility based department needs.
  • The APP will provide virtual support for Bartlett and neighboring offices with limited access.
  • On average, the expectation is four days of remote work and one day in person
  • Call structure: office call, one weekend every six weeks
  • Practice setting: Outpatient
  • Full-spectrum Family Medicine (kids-adults)
  • High quality service in one convenient location, including primary care and specialists
  • Lab testing and imaging services on site
  • Excellent office support staff
  • Board Certified Nurse Practitioner or Physician Assistant 
  • Eligible for state medical license and DEA/Controlled Substance registration

What minimum requirements you’ll need

Licensure / Certification / Registration:

  • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
  • Nurse Practitioner credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date required. 
  • Drug Enforcement Admin credentialed from the Drug Enforcement Administration preferred.
  • Registered Nurse preferred.

Education:

  • Masters Degree of Nursing required
