Registered Nurse Case Manager

Job Description

Registered Nurse Case Manager provides patient-centered, knowledge-based nursing care using telehealth technologies and care management principles to support Veterans in managing chronic, high-risk, or post-acute conditions. The RN performs comprehensive assessments, develops individualized care plans, implements clinical interventions, and evaluates patient outcomes across the continuum of care. The RN triages patient-generated telehealth data-including vital signs, symptom reports, and structured clinical questions-identifying potential exacerbations and initiating timely interventions. The RN collaborates extensively with interdisciplinary partners, including PACT, HBPC, Mental Health, Social Work, Pharmacy, and Nutrition, and facilitates communication with community providers when care is shared. The RN delivers health education, coaching, and motivational counseling to enhance Veterans' self-management skills, reduce preventable hospitalizations, and improve access to care. The RN adheres to all organizational policies, clinical guidelines, regulatory requirements, and demonstrates beginning leadership skills and sound clinical decision-making in providing safe, effective telehealth nursing care.
