What You’ll Do

  • Coordinate and oversee hospice patient care across the interdisciplinary team, including contracted providers
  • Serve as the central point of communication between physicians, caregivers, patients, and internal teams
  • Manage physician orders and ensure timely entry, approval, and alignment with the plan of care
  • Partner with Intake to support timely admissions and follow up on pending referrals
  • Continuously assess patient needs and ensure appropriate reassessment with any change in condition
  • Lead development, implementation, and updates of individualized hospice plans of care
  • Support patients and families through education, symptom management, and end-of-life care planning
  • Review clinical reports (occurrence, infection control, OnCall) and take appropriate action
  • Participate in and contribute to IDG meetings, supporting care planning and decision-making
  • Perform in-home visits as needed to support care delivery and team alignment
  • Ensure compliance with hospice regulations, documentation standards, and quality expectations
  • Identify trends or gaps in care and escalate to Branch leadership with recommendations

What We’re Looking For

  • Active RN license in state of practice
  • Graduate of an accredited nursing program (BSN preferred)
  • Previous Hospice or Home Health experience required, with strong understanding of end-of-life care
  • 2+ years of leadership, supervisory, or care coordination experience preferred
  • Proven ability to work within an interdisciplinary team environment
  • Strong clinical judgment, particularly in symptom management and patient comfort
  • Excellent communication skills - able to support patients, families, and care teams through complex and emotional situations
  • Valid driver’s license, reliable transportation, and current CPR certification
