About the Role

The ADvantage RN provides nursing assessment, care coordination, and ongoing support to members receiving services through the Oklahoma ADvantage Program.

You will serve as an important connection between the member, family, healthcare providers, caregivers, and community resources.

Your clinical knowledge and ability to identify changing needs will help ensure members receive appropriate services and can remain safely in their homes.

Key Responsibilities
  • Conduct comprehensive in-home nursing assessments
  • Evaluate members' physical, functional, and cognitive needs
  • Develop, implement, and monitor individualized care/service plans
  • Monitor changes in members' conditions and coordinate appropriate interventions
  • Coordinate services with physicians, healthcare providers, families, caregivers, and community resources
  • Provide nursing care as appropriate
  • Educate members and families regarding health conditions, disease management, safety, and available resources
  • Advocate for members and help connect them with appropriate services
  • Participate in care coordination and interdisciplinary team activities
  • Assist with transitions of care
  • Complete timely, accurate, and compliant clinical documentation
  • Maintain compliance with Oklahoma ADvantage and Medicaid requirements
  • Communicate effectively with members, families, providers, and members of the care team

What We're Looking For

We are looking for an RN who is clinically confident, organized, compassionate, and comfortable working independently.

Required Qualifications
  • Active, unrestricted Oklahoma Registered Nurse (RN) license
  • Current BLS certification
  • Valid Oklahoma driver's license
  • Reliable transportation
  • Ability to travel throughout an assigned service territory
  • Strong clinical assessment and critical-thinking skills
  • Excellent communication and interpersonal skills
  • Ability to manage time effectively and work independently
  • Ability to maintain accurate and timely documentation
Preferred Qualifications
  • Previous home health or community-based nursing experience
  • Experience with care coordination or case management
  • Knowledge of community resources
  • Previous ADvantage or Medicaid experience

ADvantage/MSU experience is preferred, but not required. We are willing to train the right candidate.
