We are a growing Oklahoma-based home health agency expanding our clinical footprint across Eastern Oklahoma. Position Overview

The Registered Nurse (RN) plays a critical role in coordinating and overseeing care for members receiving services through the ADvantage Program. This position focuses on comprehensive assessment, care planning, and ongoing case management to ensure members receive safe, effective, and individualized care in their homes.

You will collaborate closely with members, families, and interdisciplinary teams to ensure continuity of care and optimal outcomes.

Seminole, Pottawatomie, Pontotoc, Lincoln and Cleveland counties in Oklahoma

Key Responsibilities
  • Conduct comprehensive in-home assessments, including functional and cognitive evaluations
  • Develop, implement, and monitor individualized care/service plans
  • Coordinate services with healthcare providers, community resources, and support systems
  • Provide direct nursing care as needed
  • Educate members and families on disease management, safety, and available resources
  • Monitor changes in condition and initiate appropriate interventions
  • Ensure timely, accurate, and compliant documentation in accordance with agency and program requirements
  • Participate in care coordination meetings and interdisciplinary collaboration
  • Support care transitions and discharge planning when appropriate
  • Maintain compliance with Oklahoma ADvantage and Medicaid (MSU) guidelines
Qualifications
  • Active and unrestricted Oklahoma Registered Nurse (RN) license
  • Current BLS certification
  • Valid driverโ€™s license and reliable transportation
  • Ability to work independently and manage time effectively

  • Excellent communication and interpersonal abilities
  • Knowledge of community resources and care coordination practices preferred
