Job description

IMPORTANT: Applicants MUST be a Registered Nurse (RN) currently licensed in Oklahoma AND have previous supervisory/management experience in Home Health to be considered for this position.

Position Overview

We are seeking an experienced Home Health RN Clinical Manager to lead and support our clinical team while ensuring the delivery of high-quality, compliant, and compassionate care to patients in their homes.

The Clinical Manager plays a key leadership role in overseeing clinical operations, supervising field staff, maintaining regulatory compliance, coordinating patient care, and supporting quality improvement initiatives.

This is a clinical leadership position and requires an active Oklahoma RN license and prior Home Health supervisory/management experience.

Key Responsibilities

As the Home Health RN Clinical Manager, you will:

  • Direct and coordinate clinical operations while ensuring the continuity, quality, and safety of patient care in accordance with state and federal regulations and Conditions of Participation.
  • Supervise, coach, and provide clinical direction to nursing and other clinical staff.
  • Ensure services are delivered in accordance with each patient's Plan of Care and that assessments and reassessments are completed appropriately.
  • Support the hiring, development, qualification, and assignment of clinical personnel.
  • Promote the philosophy, policies, and clinical standards of the agency.
  • Monitor the Quality Assessment and Performance Improvement (QAPI) program and ensure appropriate corrective actions are implemented.
  • Monitor clinical systems and identify areas of concern or opportunities for improvement, communicating findings to the Administrator.
  • Promote compliance with Medicare, fiscal intermediary, and other third-party payer requirements through education, coaching, and oversight.
  • Participate as a member of the Interdisciplinary Group (IDG), assist with development of the IDG plan, and participate in QAPI activities.
  • Promote infection control practices and standard precautions throughout patient care operations.
  • Maintain effective communication with patients, families, physicians, clinical staff, and other members of the interdisciplinary team.
  • Meet all mandatory continuing education requirements established by the agency and applicable licensing board.
Required Qualifications

Candidates must meet the following requirements to be considered:

  • Current, active Registered Nurse (RN) license in the state of Oklahoma — REQUIRED
  • Minimum of 2 years of Home Health management/supervisory experience — REQUIRED
  • Strong knowledge of Home Health clinical operations, regulatory requirements, and standards of care
  • Demonstrated experience supervising and providing clinical direction to healthcare staff
  • Valid Driver's License
  • Valid Auto Insurance
  • Current CPR Certification

HomeCare HomeBase (HCHB) experience is preferred.

Please note: This position requires both an Oklahoma RN license and previous Home Health supervisory/management experience. Candidates who do not meet these two requirements will not be considered.
