What You'll Do

Lead Patient-Centered Hospice Care

Conduct comprehensive patient and family assessments

Develop and manage individualized, comfort-focused care plans

Deliver skilled nursing care with a focus on pain and symptom management

Serve as the Care Coordinator

Act as case manager, coordinating care across the interdisciplinary team

Collaborate with physicians, social workers, chaplains, and aides

Monitor patient condition and adjust care plans to maintain comfort

Support Patients & Families

Provide education, emotional support, and guidance throughout the journey

What You Bring

Active RN license (or compact eligibility)

Graduate of an accredited nursing program

Minimum 1 year of clinical nursing experience

Ability to manage a caseload independently

Excellent communication skills in sensitive situations

Basic EMR proficiency

Current CPR certification

Valid driver's license and reliable transportation
