REGISTERED NURSE CASE MANAGER

A Path of Care Hospice and Home Health in Weatherford, OK is seeking a fulltime RN Case Manager to provide compassionate, highquality nursing care to patients in their homes. This role is ideal for a registered nurse who wants to grow professionally while working with a dedicated, missiondriven healthcare team.

What You'll Do as an RN Case Manager

As an RN Case Manager, you ensure patients receive exceptional nursing care and support throughout their hospice journey. Your responsibilities include:

  • Completing initial assessments within 48 hours of hospice election
  • Working with the attending physician and interdisciplinary team to complete comprehensive assessments
  • Developing, coordinating, and initiating individualized plans of care
  • Conducting ongoing assessments and updating care plans based on changes in patient condition
  • Addressing physical, psychological, social, and spiritual needs
  • Communicating regularly with physicians and the care team to maintain continuity of care
  • Maintaining accurate, timely documentation to meet regulatory requirements
  • Providing leadership by instructing caregivers and volunteers and supervising LPNs and hospice aides
  • Practicing infection control and reporting any signs of abuse
  • Providing direct patient care as needed and assisting with discharge planning
  • Participating in professional development, community programs, and agency initiatives

Qualifications

  • Active Oklahoma Registered Nurse license
  • At least 1 year of clinical experience
  • Reliable transportation and vehicle liability insurance
  • Hospice, home health, critical care, or emergency room experience preferred
  • Strong interpersonal and communication skills
  • Ability to adapt to changing circumstances, mentor others, and work with compassion and patience
  • Commitment to integrity, critical thinking, and teamwork
