Thank you for your interest in providing care to a loved one. At Embracing Abilities we know how important the care of your individual(s) are. This application is for a Home and Community Support Professional (HCSP) that will work 1:1 with a consumer on the medicaid waiver. Please see below for position details.


Documentation & Requirements:

The following documents will be required to submit for the virtual onboarding process:

  • A smartphone or device with cellular and/or internet service.
  • An email address that is checked regularly as all communication will come via email.
  • Valid Drivers License - Learners Permit & State ID not accepted, must have valid drivers license with driving privileges.
  • Valid Auto Registration
  • Valid Auto Insurance Card
  • Driving Record
  • Social Security Card/Passport or Other Approved Identify Verification Document
  • Tuberculosis Screening (Negative - must be completed within the past 3 months)
  • CPR/First Aid Card (Front and Back)- Must Be An IN PERSON Certification
  • County Criminal History Record from each county of residence and or employment from the last 3 years.* (This can be obtained from the local Sheriff’s Department)
  • Background Check For Any State You Have Lived in the Past 3 Years
    • State (Limited) Criminal History Report - Indiana
  • Notes are required to be submitted within 24 hours of the end of a shift.
  • *All applicants must be at least 18 years of age or older.
  • *The above requirements are required by the State of Indiana to provide home and community support professional care per the IAC 460.

**HCSPs are responsible for any fees associated with obtaining and maintaining the above requirements.**


Training:

The Indiana Administrative Code 460 requires each Contractor/Caregiver to complete Annual In-Service Training.

The required in-service training includes, but is not limited to, the following subject matters:

•Respect and Dignity (Individual Rights, Prevention of Abuse and Neglect)

•PSCISP Training

•Communication Training

•Documentation

•Health Care Coordination (Medication Training)

•Universal Precautions

Embracing Abilities is proud to offer online training to our HCSPs. HCSPs are invited to our online training program once the initial required documentation are received and verified by our Human Resource department.


Compensation:

PAC Services:

  • $21.15/hr - Family (Legal Guardian, Parents, Legal Step-Parents of consumer who is at least 18 years of age or older) or Grandparents (consumer of any age)
  • $20.15/hr - All other caregivers

RSPO (Respite): $23.00/hr
RHS (Residential Habilitation Services): $20.50/hr
DHI (Day Hab Individual): $21.40/hr

TRNS (Transportation): $9.50 per trip - can only be billed up to 2 times per day


Please Note: All notes must be submitted within 24 hours of services being provided AND within the appropriate pay week. If notes are submitted after the payroll cut-off (Sundays) notes may not be billed to the state on time and may delay payment or not be paid to the HCSP, this is at the discretion of Embracing Abilities, per state guidelines.

Requirements:
