Location: Wayne or Macomb County within Michigan
 
Home-based | 15–20% Local Travel

Make a difference in your community! As a Behavioral Health Professional, you’ll support Medicare/Medicaid members through in-home or virtual assessments—helping improve their independence and quality of life.

Responsibilities:
  • Manage a caseload and complete member assessments in home, community, or telephonic settings.
  • Provide clinical support and collaborate with the Interdisciplinary Care Team (ICT) for members with medical complexity.
  • Develop, implement, and coordinate individualized care plans to ensure appropriate services and address barriers to care.
  • Conduct medical attestations, clinical reviews, and ensure services are provided in the least restrictive setting.
  • Support members and caregivers in accessing social, housing, educational, and other community resources.
  • Serve as the primary point of contact for the ICT and coordinate with internal and external stakeholders to meet member needs.
What You’ll Bring:
  • Michigan LBSW or LLBSW (MSW preferred)
  • 2+ years in behavioral health or clinical care management
  • Excellent communication and coordination skills
  • Valid driver’s license, reliable car, and home office with internet
